Units are billing units for this code, not a dose amount. Use a whole number from 1 to 9999. Blank or omitted units default to 1.
CSV values are exact: use Facility with a capital F or Non-facility. Amounts use plain dollars with no symbols or commas.
Set lane in each row, or leave it blank and choose Default lane once above the results. A row's lane overrides the default.
